We are changing the way we deliver care for you! Here’s how:
The physicians of the Grandin Medical Clinic are excited to participate in a new Alberta Health program that will improve the way we offer care. This new model allows us to care for our patients more proactively, involving our entire health care team, including Physicians, Registered Nurses and Pharmacist.

Before visiting an alternate Physician or walk-in clinic for your immediate healthcare needs, give us a call to allow us to offer you all options available at the Grandin Medical Clinic. 

We’re working very hard to improve wait times for our patients to see our healthcare team. 


How will this change affect me as a patient?
As our patient, you will continue to receive excellent care from your family doctor. Our goal is to put a greater emphasis on patient education, health promotion, and wellness. A few other changes you may notice include:

  • Our team will continue to develop more ways for you to access care
  • Our nursing and pharmacy team will be more involved in caring for you 
  • You will receive a greater range of services, and develop a close relationship with your doctor and other health care providers

As a result, you’ll have access to more support from our entire team of health professionals throughout your health care journey.  


What does this mean for me?
The affiliation form outlines details of the patient-doctor relationship and asks patients to seek care at the clinic where they have signed up. This allows your family doctor to have a complete understanding of your healthcare needs and creates a trusting and well-informed relationship between you and your family doctor.


What do I need to do now?
To participate in this program, we are required by Alberta Health to maintain an accurate and up-to-date list of all of our valued patients.

Both you and your doctor need to sign the affiliation form agreeing to a patient-doctor relationship and the expectations and benefits related to the relationship. Once your affiliation form is signed and returned to the clinic, you have been connected to Grandin Clinic.  

By signing the affiliation form, you agree to seek care whenever possible at the clinic you sign up with and update the clinic of any changes in your health.

You can only be affiliated with one clinic at a time. If you sign up with a second clinic, you are automatically removed from the first clinic’s affiliation list.
